Simply call our office at (859) 986 - 4661. You can fill out the New Patient Form to bring to your initial appointment.

Emergency?

Dental problems don't always happen during normal business hours.

Should you require emergency dental service, please contact our office for emergency contact information after-hours.

Insurances Accepted

We are ‘in network’ for most insurance companies. The best way to be sure that Maxam Dental is in network with your insurance provider is to get in touch with us and ask directly. We will work to accommodate you to the best of our capabilities. (accept Wellcare and Aetna Better Health for age 18 and under patients; Kleer plan is available for all patients without insurance)
